Because the game spans such a long timescale, and large scale conquering is restricted early on, the smaller nations still have a chance to expand or develop in other ways. Anything from a One Province Minor to the Ottoman or Chinese Empire will all play extremely differently. The best thing is the choice in both starting size and location. A lot of the DLC I'd probably never buy, so I wouldn't worry too much about the $300 thing. I did eventually pick up Emperor, Mandate of Heaven and Rule Britannia on a 50% for the Irish and European mission trees and the extra flavour details for Japan and China.
